Utah Governor’s Office of Economic Development

All contestants are required to have a current (less than 6 months old) HIV, Hepatitis B/C tests (less than 1 year old) prior to their contest.  A negative test for Hepatitis B and proof of vaccination/immunity for Hepatitis B will permamently fulfil the Hepatis B requirement.  Contestants 36 years of age and older also are required to have a 12-lead EKG on file that is less than 2 years old and reviewed by a physician.

All contestant are required to have a physical examination completed by a physician (M.D. or D.O.) prior to licensing or relicensing.  The commission-approved form is available below.  However, the commission will accept essentially equivalent medical examinations on forms approved by other Association of Boxing Commission member commissions.
